Appointment requests
Sending a request through this site asks us to contact you. It does not confirm an appointment. A member of our staff will call you to schedule, usually within one business day.
If you have not heard from us, please call the office rather than assume the request went through.
Accuracy
We work to keep hours, locations, accepted insurance plans and service information up to date, but details change. Call us to confirm anything you are relying on — particularly insurance coverage, which changes frequently and depends on your specific plan.
