Chronic condition care
Long-term care for diabetes, blood pressure, cholesterol, kidney, heart and lung conditions in Upland and Rancho Cucamonga.
- Diabetes careWe help you manage diabetes over the long term — checking your A1C, reviewing your medications, watching your kidneys, eyes, and feet, and adjusting the plan as your life changes. Most patients are seen every three to six months once things are steady.
- High blood pressureWe diagnose and manage high blood pressure — confirming it with accurate readings, finding a medication that works without making you feel worse, and checking in regularly. High blood pressure has almost no symptoms, which is why routine visits matter more here than in almost anything else.
- High cholesterolWe check and manage cholesterol, and more importantly we put the number in context. Cholesterol matters because of what it does to your heart and arteries over decades — so the decision about treatment depends on your overall risk, not on one figure in isolation.
- Kidney diseaseWe monitor and manage chronic kidney disease. Most kidney damage is caused by diabetes and high blood pressure, so the main work is controlling those tightly, reviewing medications that strain the kidneys, and tracking function over time so changes are caught early.
- Heart failureWe manage congestive heart failure day to day — getting your medications right, watching fluid, and catching build-up early. Most heart failure hospital admissions are preceded by several days of gradual weight gain, which is exactly what daily weighing is designed to catch.
- COPD & chronic lung diseaseWe manage COPD and other long-term lung conditions — getting inhalers right, preventing flare-ups, keeping vaccinations current, and giving you a written plan for when breathing worsens. Stopping smoking remains the only thing that changes the course of the disease, and we help with that properly.
- AsthmaWe diagnose and manage asthma long term. The goal is control: no waking at night, no limits on what you can do, and rarely needing your rescue inhaler. If you are using a rescue inhaler more than twice a week, your asthma is not controlled — and that is fixable.
- Liver healthWe investigate and manage liver problems, most commonly fatty liver disease. An abnormal liver blood test is common and usually not alarming, but it should be explained rather than ignored — and early fatty liver is one of the few chronic conditions that can genuinely be reversed.
- Digestive & stomach problemsWe assess and manage ongoing digestive problems — reflux, indigestion, bloating, constipation and bowel changes. Most turn out to be common and manageable conditions, but a few symptoms need ruling out first, and that is what the first visit is for.
- Thyroid & hormone conditionsWe test for and manage thyroid and other hormone conditions. Thyroid problems are common, easy to test for with a single blood test, and highly treatable — and their symptoms are vague enough that they are frequently put down to stress, ageing or low mood for years first.
- Anemia & blood conditionsWe investigate and treat anemia and other blood count abnormalities. Anemia is not a diagnosis in itself — it is a sign that something else is going on. So the work is finding the cause, not simply prescribing iron and rechecking the number later.
