What this page covers
This policy explains what happens to information you provide through this
website — elitefamilymedical.org. It does not cover the information we collect
when you are a patient at one of our offices. That is governed by our Notice of
Privacy Practices, which we give you at your first visit and which you can ask
for at any time.
What we collect through this site
When you book an appointment. Booking is handled by our scheduling provider, Tebra, through a window embedded on our booking page. What you type there goes to Tebra, not to this website. This website itself has no forms and collects nothing you type.
Please do not send medical information to us by email or through any online form. If you need to discuss anything clinical, call the office or use the patient portal.
When you browse. Like most websites, we collect basic analytics — pages visited, approximate location, browser type, and how you arrived. This is used to understand which pages are useful, not to identify you individually.
What we do not do
- We do not sell your information.
- We do not share your phone number or personal details with third parties for marketing or promotional purposes.
- We do not use information submitted here for advertising targeting.
Cookies and analytics
We do not run analytics on this website, and we do not track you across other sites. Nothing on this site reports your behaviour back to the practice.
One piece of embedded content — the Google reviews panel on our home page — is provided by a company called Elfsight, and it may set its own cookies when it loads. We receive no information from it. See the list of embedded services below.
Some third-party content embedded on our pages (see below) may set its own cookies once it loads. Those are covered by that company’s policy, not ours.
If we add analytics later, this section will be updated first and a way to decline non-essential cookies will be provided at the same time.
Text messages
This website does not sign you up for text messages. It has no forms at all, so there is nothing here that could ask for or record consent to be texted.
If the practice texts you about an appointment, that consent is taken separately — in the office or over the phone — not here. In that case: message and data rates may apply, frequency varies, and you can opt out at any time by replying STOP. Mobile information and text-messaging consent are never shared with third parties or affiliates for marketing purposes.
Your choices
You can ask us at any time to correct or delete the contact information you gave us through this website. Call the office and we will take care of it.
For your medical records, a separate process applies under HIPAA — ask the front desk and we will explain how to make a records request.
Third-party services
Some pages load content from other companies. When that content loads, those companies can see your IP address and basic browser information, and may set their own cookies. Each has its own privacy policy.
